WAIVER AND RELEASE OF LIABILITY

I, the undersigned, wish to participate in the martial arts classes, training sessions, and related activities offered by 720 Martial Arts Academy. In consideration for being permitted to participate in these activities, I hereby acknowledge and agree to the following:

1. Assumption of Risk: I understand that participating in martial arts activities involves inherent risks, including but not limited to the risk of injury, physical strain, and emotional stress. I voluntarily assume all such risks.
2. Physical Condition: I represent that I am in good physical condition and have no medical condition that would prevent my participation in martial arts activities. I agree to inform the instructors of any changes to my health or physical condition.
3. Release of Liability: I release 720 Martial Arts Academy, its instructors, staff, and any affiliated entities from any and all liability for any injury, loss, or damage arising out of or connected with my participation in martial arts activities, even if such injury, loss, or damage is caused by the negligence of 720 Martial Arts Academy or its representatives.
4. Photographic Release: I grant 720 Martial Arts Academy the right to use photographs and/or videos of me taken during martial arts activities for promotional and marketing purposes.
5. Emergency Medical Treatment: In the event of any medical emergency, I authorize 720 Martial Arts Academy and its representatives to obtain medical treatment for me if I am unable to do so myself.
6. Compliance with Rules: I agree to comply with all rules and regulations of 720 Martial Arts Academy and to follow the instructions of the instructors.
7. Responsibility for Damages: I agree to be financially responsible for any damages to the facilities, equipment, or property of 720 Martial Arts Academy caused by my willful or negligent actions. In the event of any such damages, I understand that 720 Martial Arts Academy may charge me for the cost of repair or replacement. I authorize 720 Martial Arts Academy to charge my credit card or any other payment method on file for the amount owed for damages. If payment is not made within 30 days of receiving notice of the damages and associated costs, I understand that 720 Martial Arts Academy may take legal action to recover the amount owed.

I have read and understand this Waiver and Release of Liability, and I voluntarily sign it with full knowledge of its significance.
